Kernel parameters. Brandon posted a detailed answer here. Don't forget to reboot after changing the values.
Wrong filesystem permissions after installation. All files and directories under /var/mqm and /opt/mqm should be owned by user mqm and group mqm. Fixed by issuing the following command under root: 'chown -R mqm:mqm /var/mqm/* /opt/mqm/*'
/var/mqm is out of space.
If all else fails:
Check the *.FDC files at /var/mqm/errors for detailes.
Run crtmqm under truss to see if any system calls fail.
Try running crtmqm as root.

Do not run crtmqm as root, if root is not in the MQ group. You'll have semaphores / shared memory left around that is owned by root and can cause problems in the future (especially if you try to do things as mqm later on).
