I would like to look for a way to monitor a queue, logging it, what connections made it to the queue, when and what actions did it take
Is it possible?

What have you done so far? I've posted lots of sample MQ/PCF programs on my blog that do these sorts of things.

MQ explorer with support pac MS0P added is pretty easy to use.

Well, I don't think there is any question about it. It can only be attributable to human error. This sort of thing has cropped up before, and it has always been due to human error.

The Application Activity Trace that comes with IBM MQ is another tool to review to see if it meets your requirements.
