I tried searching for MQSETMP. I found a link for Cobol invocation in WMQ. I am not sure, if I need this.

If you're using WMQv7 then you should use this in preference to an RFH2. If you're not unsing WMQv7 it's not available. Finding a link on the Internet does nothing to demonstrate availability on your site, or (given that you're using z/OS) the specific queue manager you're using.

As far as alignment is concerned, I have tried various permutation/combinations.

Do. Or do not. There is no try. Sequence dialing through all the permutations until you hit one that works is not going to result in a supportable configuration.

I could be wrong in alignment though.

Yes you could.

I am not able to find the specific layout for cobol,z/os for RFH2. Is it any specific structure I need to follow?

Yes. It's the same specific structure that an RFH2 has on every platform. The only variable is how the different programming languages support the data items (you'd be doing something different if you were producing an RFH2 using C on z/OS).

And in the 2 years since this thread was last active, my advice has become even more relevant (as WMQv7 is now prevelant).
